Anyone living in our Practice area is welcome to register with the Practice You can bring your medical card to the reception but dont worry if you have lost it You will be asked to fill in a simple form asking for you current and previous details You will also be asked to fill in a medical questionnaire which helps us until your medical records arrive from your previous doctor You will also be invited for a registration health check either with your doctor or with a Practice Nurse
In theory you can register with any one of the Doctors here in the Practice but we cannot specify in advance which Doctor you will be registered with If you wish to discuss any aspect of registration then Kamal Bahia our Practice Manager will be pleased to answer your queries
